  • Billing end date will be 60 days from when this form is submitted.
    If the disconnect date needs to be postponed please inform Dynalink 7 days prior to the original disconnect date.
    You will be charged 60 (sixty) days from the day you provide Dynalink with the new cancellation date.
    Term charges will apply if the contractual period has not been satisfied.
